Before they check you or even ask what you need or want, you are required to sign away all your rights. If you wanted to get A but you got B instead, that's not their problem! If their treatment directly caused you permanent injuries, hey that's just too bad! Not a single line in their legally-binding agreement considers the patient's right to get what is agreed upon. Rather than express an intent to deliver, It is about covering their backs against malpractice. Been to dentists in 3 continents, never came across anything quite like this. I declined to sign and walked right out.
